Abstract

Stroke is one of the non-communicable diseases that remains a leading cause of mortality and disability worldwide, including in Indonesia. The increasing prevalence of stroke and the high number of Years Lived with Disability (YLD) indicate the substantial health burden caused by this disease. This study aimed to analyze the trends in stroke prevalence and YLD in Indonesia during the period 2021–2023 based on data from the Global Burden of Disease (GBD). This research employed a quantitative descriptive method with an ecological approach and time-trend analysis. Secondary data were obtained from the GBD Results Tool platform and analyzed using R software version 4.4.1. The analysis was conducted based on geographical regions, particularly across the major islands of Indonesia. The results showed that both stroke prevalence and YLD gradually increased during the 2021–2023 period. Java Island recorded the highest prevalence and YLD compared to other regions, while Maluku and Papua exhibited lower prevalence but greater variation in YLD. The increase in prevalence and YLD was associated with high urbanization, unhealthy lifestyles, hypertension, diabetes, and unequal access to healthcare and rehabilitation services. Pearson correlation analysis demonstrated a very strong relationship between stroke prevalence and YLD in each study year (r > 0.99). In addition, significant regional differences were identified in the distribution of stroke prevalence and YLD. This study concludes that the burden of stroke in Indonesia continues to increase and demonstrates regional disparities. Therefore, health policies should focus more on controlling major risk factors, improving equitable access to healthcare services, and strengthening post-stroke rehabilitation programs to reduce disability caused by stroke in Indonesia.
