Rapid and accurate diagnosis is essential to reduce transmission and improve treatment outcomes. Molecular Rapid Test (MRT) or GeneXpert MTB/RIF has been widely implemented as a diagnostic tool capable of detecting Mycobacterium tuberculosis and rifampicin resistance simultaneously within a short time. This study aimed to describe the results of Molecular Rapid Test examinations among presumptive tuberculosis patients at RSUD Prambanan, Sleman Regency. This research used a descriptive observational design with secondary data obtained from TB 04 records and the Tuberculosis Information System (SITB) during January–December 2025. A total of 2,384 samples fulfilled the inclusion criteria and were analyzed descriptively using frequency distribution and percentages. The results showed that most patients belonged to the low-risk category (96.9%), while only 3.1% were classified as high-risk. Most examination results indicated MTB not detected in both groups. Rifampicin-sensitive MTB was identified in 153 patients, whereas rifampicin-resistant MTB was found in only one patient. Positive MTB results were more frequently identified in male patients and adult-to-elderly age groups. The study concludes that most presumptive tuberculosis patients examined using GeneXpert showed MTB not detected results with a very low prevalence of rifampicin resistance. These findings support the importance of maintaining early detection programs and strengthening tuberculosis control strategies in healthcare facilities.                                                               ER -
